Notes: November 9, 2022 -- Ampio Pharmaceuticals, Inc. (the “Company”) amended its certificate of incorporation to implement a 15-to-1 reverse stock split. As a result of the reverse stock split, at 4:01 p.m. Eastern Time on November 9, 2022, every 15 shares of common stock then issued and outstanding automatically were combined into one share of common stock, with no change in the $0.0001 par value per share. No fractional shares will be outstanding following the reverse stock split. Holders of fractional shares will be entitled to receive, in lieu of any fractional share, the number of shares rounded up to the next whole number. Immediately prior to the reverse stock split, the Company had 226,286,867 shares of common stock outstanding, which will be proportionately reduced for the 15-to-1 reverse stock split.
